1.  What immunizations or shots does my child need to attend school?

New York State requires the following immunizations or shots for school attendance:

        DPT/DTAP - 3 doses ( 2 boosters are also recommended)

        Polio - 3 doses (1 booster is also recommended)

        MMR - 2 doses

        Hepatitis B - 3 doses

        Varicella/Varivax - 1 dose (or Doctor documented evidence of disease)

2.  Does my child need a physical examination?

Physical exams are required in Pre-K or Kindergarten, 2nd Grade and 4th Grade.

Physicals by your own doctor or clinic are encouraged. 

Physicals will be done by the school doctor for students who have not provided an exam form from their own doctor or clinic.  Notices will be sent to parents prior to physicals done by the school doctor.

3.  What do I do if my child has head lice?

Please notify the Health Office if you find head lice on your child.  We are glad to answer questions to help you take care of your child. Use a lice treatment on your child and remove all nits.  Check other family members.  Please bring your child to the Health Office before their return to the classroom.

From time to time the students are checked in school.  If any student is found to have live lice the parent is notified and will need to come after their child.  If nits only are found generally a note is sent home at the end of the day.  Please remove all nits before returning with your child to school.  Again your child will need to be checked in the Health Office before their return to the classroom.

4.  What happens if my child is sick ?

Please keep your child home until their temperature is under 100 degrees for 24 hours and vomiting and diarrhea have stopped for 24 hours.  If your child has an eye infection, unknown rash or harsh, persistent cough they should be cleared by your doctor before returning to school.

5.  Are medications allowed in school?

A doctor's order is needed for all medications in school including over-the-counter medications such as cough drops and cough syrup.  The order must also be signed by the parent/guardian.  Medications are supplied by the parent/guardian.

6.  What health screenings are performed in school?

Basic hearing and vision screenings are performed on all students at some point during the school year.  Near point vision and color perception are also checked at various times.  Fifth Grade students are screened for scoliosis.  A referral form will be sent if there is a concern with any of these screenings.
